How to rescue operative?
I clicked on her portrait which is behind bars. On the bottom it states that she has been captured and needs to be rescued if not, countdown of days left before death. I see no mission or button to rescue her.

Anyone having issues rescuing operatives despite doing so successfully? In recent days I have not successfully rescued a single operative despite it telling me it was successful.

Let's say your operative is captured in Berlin. You have to take another spy to build a new spy network in Berlin. About 20 days you will see next to his name rescue operative.

Right, this is other way how reach the mission as they appear as icons at place of your network. However it is more convenient just open the list in agency tab(at least for me) than scroll over countries on map.

no. Never observed anything like that.
Turned out to be an issue with the Expert AI mod.
