It took me two playthroughs and a lot of savescumming.
First time I played blind for fun (on normal). The second time I rushed to get all the remaining achievements (on hard). 280 hours total. But I bet it can be done much quicker. I just took my time and enjoyed it.
It cannot be done on one hardcore playthrough. And even if it could be, I wouldn't recommend trying.
Back up your saves regularly. The game deletes some old saves automatically. So this way you'll keep access to any part of the game if needed.
There are some problematic achievements:
Fighter, Casanova, Plaque Doctor, You had one job!, Trial and Error, Cleric's pet and absolute pain in the ass Full House Sinner.
I would recommend this: if someone is dying, leave everything else and help him right away. Also party hard with Father Godwin to avoid later troubles. And read names on crosses beside the road.
Beware, some quests are timed and the game does not tell you directly. So listen carefully.

Then another as Hardcore, this time perhaps not only to do Hardware and beat the game under that but as to be a Virgin the entire game and also not Learn to Read. It helps to play it so that the achievement for going through the game without learning to read comes in a 2nd playthrough so that you've done the stuff that requires reading before-hand, thus have better understanding of what to do when you can't read in another playthrough.
Obviously you can't court Theresa, Stephanie and pay for wenches @ bathhouse and still remain a virgin the whole game.
And best bet for a game start is when speaking with Henry's Mother at the very beginning; pick the two options for SPEECH, since this is needed and harder to level up quickly. Others like Vitality or Strength can be leveled up rather quickly. Especially once you have option to pick a perk from Herbalist that can help level up Strength.
You can level up Vitality rather quick just from sprinting for the entire period your stamina bar remains. Then stop sprinting just before it runs out.
also the stats do feel easy to level up and then there is speech.

If you played on normal, just load your save and hunt down all remaining achievements, except: hardcore, merciful, tis but a scratch and virgin. You'll have to do them in a separate run, but in that run you'll just focus on the main quest line.
Trial and Error (Pribyslavitz) is a pain but can be done quite quickly. Just reload save before learning how to read, defeat Runt and rebuild the city either with Skalitz bandits abuse and/or Peshek chest trick.
I agree that downloading other save files feels like cheating and pointless effort.
That said, hunting of achievements is also pain and not fun in some cases, so maybe it is unreasonable after all :D
Yes, for humanoid NPCs. Nothing against animals. As there are plenty of enemies in the game you can avoid or simply knock-out. Most killings can vmbe avoided. And that only pertains to Henry not the playable characters within the various DLCs
