I finished the mass fusion quest wich made it impossible to do one brotherhood quest because I was missing an item. I reloaded a save where I alredy had that item and didn´t gave it to the institut

and weakness at the same time of the game is the way the SS obtains information. The goal of most players is usually to race through quests. So they miss key clues from terminals and documents
the information has an opportunity to sink in and then you start to understand why.

Vats works off line of sight, line of sight is calculated from the tip of your gun to the enemy's body part you are targetting at the moment of executing the order not at the moment of you giving it
