Install game on an SSD not HDD. Get more memory/RAM as well. Disable any hi res textures.

How many mods did you install? Having too many mods can also slow the game down.

And some parts downtown just have many vertices and refreshing the screen is slow on older slower PCs.

Maybe defrag your HDD too.

Are you on Windows or something else? Do a disk check in Windows.

If the game is on c: do this in a DOS box: chkdsk c: /f

to fix any errors. This could take a while.

posisbilities:
-Google OneDrive ( unistall it!)
- Windows driver update
- anti virus

Playing while completely disconnected from the internet often alleviates these issues.
I have a robusr rig now ( 4070, 64GB RAM, SSD, i90 10900KF slightly overclocked) but back when I didnt i got a lot of choppiness and 90% of ti went away when I disconnected from the internet.
yes u can play with Vortex and no steam connection.

Quite a few mods, most of them are bug fixes or slight retextures.
no matter where i am (Railroad, institute, downtown, diamond city, dugout inn, sanctuary) its always choppy and laggy to the point it's unplayable.
I am on windows.

Is there a lot of gun combat occurring near you and do you have weapon debris turned on? I use Nvidia and sudden & prolonged frame drops to nearly unplayable levels were caused by weapon debris. Shooting at puddles of flammables or being shot at by enemies, where you see chunks get broken up was what started a low frame moment. Shadow distance on high/ultra as well will bring on stuttering and lower frames in Lexington areas and Boston downtown. Medium shadow distance and quality smooths every areas performance.

I'm having the same problem. It must be related to the Steam update from the 12th. Did you figure anything out?

I also run a NVIDIA 4070 but no issues.
But my steam updates are turned off.

Nope. I'm thinking of deleting and reinstalling fallout

Same. I purged all of my mods and started a new vanilla game but it was still unplayable.

Did you turn off Weapon Debris? I think it is "incompatible" with new cards 3000 series and above.

It crashes on mine when I turn it on.

Is this a new thing since the 11th? Because my game was working just fine until then and i made no changes to anything.

I deleted all of my mods, uninstalled Fallout 4, deleted Fallout folders, then reinstalled. Launched a vanilla game and it's still ♥♥♥♥♥♥. I don't know what else to do. I am sad.

Using Nvidia settings? Have you flicked the setting on that says "Let the application decide" under Manage 3D Settings?

I don't use Nvidia settings for applications and I keep up to date with Game Ready driver as well. For things to all of a sudden happen perhaps you could check the setting and or update driver. As a fellow Nvidia GPU user I can say the games runs best on SSAO, medium shadow quality/distance. Regardless of how high end or low end your card is.

HBAO and weapon debris turned off or you will experience intermittent low frame times and then intermittent crashing due to HBAO. HD Texture pack from bethesda runs fine and there is no real disadvantage to using it, besides my belief it causes texture pop out or unloaded models in the map. But I removed it and I think the game turned out more stable all around. Never does the game go into low frames unless I was to turned shadow distance to high and look at the right angles in Lexington and Boston downtown.

I don't know what else to do.

No, Weapon Debris has been a thing since the game was released. If it was working before, you had it turned off. (Or it would have crashed constantly)

Since you had mods before, did you clear out any stray .ini's left by MO2 (If you use it) and your Fallout 4 folder in My Documents?
Stray .ini files left over can cause all kinds of problems.

Did you make sure to toggle Weapon Debris + God Rays off after you re-installed? Double check they are fully off.

Is the game running at 60fps? If it's laggy and slow, make sure the game isn't capped to somthing silly like 24fps or 48fps, as the game can cap it depending on your monitor refresh. The higher the native refresh, the bigger the divide, so 144hz becomes 48fps, and 72hz can cap at just 36fps.

You'll want to cap it to 60fps when you change this line in your Fallout4.ini found in your My Documents folder, found under the -display- header;

iPresentInterval=1

Change the 1 (Or whatever number is here. Could be 2 or 3 as well) to a 0, and then manually cap the game via Nvidia control panel/AMD Adrenaline (Depending on the card manufacturer your using) to 60fps or below.
Hey thank you for caring. I went ahead and changed iPresentInterval=0. I'm getting 1 FPS (and it's not even that). My game is TOTALLY borked and unplayable!

The opening cinematic plays fine but then when i get to the mirror scene it takes minutes to even say War Never Changes we are lagging so bad. I have no idea what this could be. Like I was saying, every thing worked flawlessly 4 days ago and i haven't changed a thing.
