what are your pc-specs and what monitor-resolution you play the game?
Remove the texture pack, and see if that makes a difference. Also, do you have any character "beauty" mods or body mods? If so, make sure those are installed correctly as well.... ESPECIALLY "hi poly" or "2k face" mods....
As mentioned before the HD DLC is not worth it. If you want a higher resolution and stay vanilla use this instead:

Download all 4 files in the files section and the cubemap fix mentioned in the mod description. The also mentioned landscape mods are not needed. This should also solve your black face problem since that mostly happens when face texture and other character assets don't have matching resolutions (google will help you there - fallout 4 black face bug).
