also are you using mod organiser? I had plenty of issues when it came to changing ini settings since i found out that they tend to reset to their old setting or not work at all and you might have to make the ini file a read only file after you change it etc etc until I tried it with my mod organiser.
maybe setting ipresentinterval to 0 might help
certain mods can also eat away fps like scrap everything which is a mod that i couldnt live without and had no choice but to delete and my failure to find something similar and a bit of OCD brought me back to it.
2) Enable Fast Sync (nvidia), Enhanced Sync (AMD).
3) Set your monitor to 144hz.
4) Set your FPS cap to 144.

In both files the value is set to 1. :(
I run Nvidia. There's no Vsync option for Fallout 4 ingame nor in the launcher, that's why I used the control panel.
Monitor is set to 144. Did cap on Nvidia panel to 60 even, cause I don't need the game to run on more than 60fps tbh.

it needs to be zero in both ini files, else it can block some control panel settings
cap the fps to 60 in the nvidia control panel for fallout4.exe
