I can't think of any reason why it wouldn't.
I'm guessing there won't be much flexibility in the intro (aside from a few dialogue options). There will almost certainly be an alternate start mod.
Starting in the vault 111 or another place should not matter much.
Even if it did.
The new mod could just say they dragged your "lifeless" corpse out of the vault and you woke up another place.
There will always be a way.
However. Something for fallout 5.
Different "races" and factions.
You can chose the lone wanderer and start in a vault. Or you can chose to be a ghoul and start another place with radiation and the likes.
Or a bandit and start the game in a bandit camp.
Different starting zones kinda like you have in world of warcraft.

they can do that in 4 too
It would be cool, but I understand why they tend to stick with one beginning, since it's a lot easier to tell a story if you only have to work with one intro.
I'd be genius if bestheda has "multiple backgrounds" we can shoose from when starting the game. That be pretty neat. Our background has an effect on how people reacted to you in the the wasteland would be pretty nice.
