You will require multiple games however (or a save made before you are forced to commit to a side), as each of the main factions has their own set of achievements. So in that was some of the achievements are missable in a single game.
While you CAN make a manual save at that point and choose different results, that will still mean you have about 100+ hours of any given faction if you do their quests to run through if you really want to 'do all of it' on one character.
The game is not meant to have 100% achievements from ONE character. It's got so many different variations on character building, armor and weaponry, companion choices, and faction choices, that it's simply : make another character. Dedicate your run to one faction, and try the others with different characters and play styles.
You can progress all 4 factions untill that point.
