Since this is obviously not the case, either your assumption that everyone plays FO4 using mods is wrong or the claim that FO4 + NG patch + mods => unplayable is wrong.
Beyond that, I agree barely anyone plays this games without any sort of mods, is not even an argument.
Plenty of people play without mods, it just doesn't usually come up outside of discussions about it. There may also be some degree of bias against seeing it since having fewer mods also means having fewer things to break and require forum assistance.
Personally, unless you count Creation Club free items, the only modifications I've got are a couple of tweaks to INI file settings.
Save for the fact no one has any exact number of the exact number of players who don’t mod.
I do not like taking risks that can ruin the things I own.
I admit there is fun to be had with modding but the risks simply aren’t worth it.
And yet, actual mod makers are doing just fine. Never seen you here in the forum asking for any help. Bethesda doesn't read this forum, if you have genuine complaints about *the game* (and not 'the mods!!' because they don't own and cannot help you with those) then you should email them.
