you get it from a mission from a guy at the railroad, follow the freedom trail to get to them.

I wish it was a Makarov, but it's actually modeled after a Walther PPK, the gun itself and the entire mission is pretty much a James Bond reference. It's a unique 10mm pistol, called the Deliverer.

The mission that allows you to get the gun involves working for the Railroad and you can start it by listening to a conversation between two men at the back of Diamond City. One of them will mention Railroad and the Trail of Freedom, giving you the quest.
