Here's to hoping it doesn't pop up (too much!) again!
I haven't seen that but I'm seeing this almost constantly now, and I don't know why. Also sometimes the ground textures look like...well some kind of sponge and raisin pudding, to be honest. Warped and stretched, not so much from textures that haven't loaded, just...horribleness. It fixes itself upon quicksave/quickload though. Unlike those white loading screen models.
Also, I do believe that's the ghost of that one super mutant you killed in that one fight in that one place, on that one day you did the thing with the things. He's totally haunting you for it.
I got a super beefy system, and this has been happening to me for at least the last two nvidea drivers. Dunno if it was a problem or not before the last two updates cause I was only just starting the game then.
The original problem is just LOD Objects not fading out when they're supposed to. It's a problem with the game itself.
The second "problem" is because the poster pushed the Vats button in a loading screen. It's a feature.
The most important thing to know is how to solve this problem but I'm guessing from the post above me that there's no solution currently.
Sadly, I can't seem to remember how I've solved this problem, but it hasn't happened for me in ages. From what I've gathered (IIRC) the bug has to do with the game itself not unloading LOD meshes, as 'Bansheebot' stated just above.
Well, I'm sorry I can't be of much help, though I will ask this on a limb: are you running the game above 60 frames per second? I don't *believe* that would contribute to this problem, especially since when it happened to me I had it capped to 60, but anything above 60 really screws with the game's logic.

Some of the current graphical glitches are the result of Bethesda downgrading the graphics.
> Change Detail to Low
> Go to Advanced options
> Manually change all settings back to Ultra
I can confirm that this works, at least for me, Thanks man, if there's anyone else who has this problem on this thread, try this if it works then mark it solved so people can find it easier.
