wear gas mask with helmet
Gas mask should be treated as face mask and worn with metal helmet. Consider WWI.
But in fallout 4 I could only wear healmet without gas mask or gas mask without healmet?

Modders shouldn't have to do this' the wear armour icon has a helmet and mask on lol' Forgot along with 3rd person holster weapons.

2015 reversing typical detail expected to be kept.

unfortunately you'll have to mod it. you're in luck, theres already one on the nexus, More Armor Slots or something along those lines. allows you to do what you're looking to.
