Even when you prime them or start the throw animation before entering VATS, they just disappear and nothing happens. Worst "feature" of the game so far, I loved the slo-mo VATS explosions, it feels real lacking when you can't use all of your weapons in that mode, especially when you could before. It's because they're now "offhand" items and melee weps and guns have priority.
I doubt Bethesda will do anything about this so we'll have to wait for the mod tools next year.
If you're good at timing it, I suppose you could throw a grenade and then wait for it to get airborne so it doesn't disappear on you and then pull up VATS, too. There's often a delay for it to appear so that could be tougher than shooting a molotov out of an enemy's hand.
It's finicky either way. There should be a button or key to throw your offhand grenade, and it should take x amount of AP to do so like anything else. Or you should be able to equip your grenades in the main hand just like before.
Without releasing the button, enter VATS mode.
