2. Alt-tab after launching the game and see if Windows popped up a message saying your firewall is blocking this game from accessing the network. If so, allow the game to access your private network. I am on Windows 7 and I initially had this exact problem - I just allowed access and problem solved!
Also, ensure your phone/tablet is on Wi-Fi and that your PC is on the same LAN (wired or wireless doesn't matter, but the router must be the same). It won't work over data.
2 - Turn the Pip Boy App thing off and then on again in the options menu, and close/launch the game. Wait a minute or two, and then try connecting your phone to it again.
3 - Make sure your phone is connected to your local wi-fi network. Your PC and phone have to be on the same wi-fi network for the app to find your game. That's how the app recognizes the game. Fallout 4 sends a signal via your local wi-fi when you have the app enabled in the options menu, and the app picks it up and displays the information the game sends via wi-fi.
4 - Check in your router settings and see if its activity log indicates that it's blocking a signal from either your PC, your phone, or Fallout 4 itself.
As a note: You don't even have to load a save; you can connect the app while you're in the main menu. I was able to get the app connected in the main menu, and it will just have a message saying that there's no game loaded. As long as it doesn't tell you that it couldn't connect, it's working, and it'll load up your Pip Boy when you get into the game.
Another note: You don't even have to leave the vault, or have a Pip Boy on your character. You can connect the app at the very beginning of the game before you get the Pip Boy or leave the vault, and it will have a different message displayed saying that you don't have a Pip Boy. The moment the scripted event for exiting plays, your Pip Boy will pop up on your phone. Pretty neat actually, how seamless it is.

We've come a long way since the Gamecube-to-GBA-link days (the first time I ever saw this sort of feature, on Splinter Cell).
After fiddling for a while, including reading this thread, I got it to connect.
I was trying to connect it while I was in the game. Apparently that was a no-go. The answer (in my case) was to be in the main menu before you load a game, then tell it to connect. It immediately found the session, but told me that I wasn't in a game... and it was correct. When I hopped into the game, it connected just fine.
I'm going to try it on my iPad next.
I can't get it to find my game..
Check your firewall settings. If Fallout 4 can't access your private network, the pipboy app can't find your game.
My PC is also connected via wired while my phone and tablet are on wifi. I initially couldn't connect to the app until I allowed Fallout 4 through my firewall.
I'm having the same issue MythrylFist. Made sure it was allowed, though I'm not actively running avast or anything like that. Let us know if you find another issue to be solved that I might be missing.
I've given it a minute to refresh before restarting the game and cannot see anything blocking my signal.
