That image isn't cryptic in the least, it has been around since Fallout 3 and was part of a Vault Survival Guide.

What is the spoiler? It is known since Fallout 1 that ghouls were normal people which were under heavy radiation and the one that survived that became ghouls over a long time with a prolonged life and maybe some healing, thoughness effects...Afair Harold told you that in FO1 and in Fallout 3 you could see it with Moira after detonating Megatown.

The new perk is called Ghoulish, in which it lets you restore health from radiation and makes random feral ghouls friendly. It does not turn you into a ghoul.

And the picture shows you as a ghoul so there is still a chance it does. Maybe they want people to wonder that's why they are so vague in the description and not saying outright. If I'm wrong I'll admit it?

What if... After X amount of radiation healing... you become a ghoul ?.. That would be epicsome.
