"Skillbooks" now give unique perks when you acquire them. For instance, the Grognak the Barbarian books give and rank up a perk that affects melee combat. Bobbleheads haven't been stated to do anything except there's two achievements for them, like Fallout 3.
I'm pretty sure you need to spend perk points to level it further, they don't go up simply because you achieved a certain level.
Though you do need to be x level in order to get rank 3 of x perk.

Yes, that is how it works. Higher ranks of perks have level requirements that you must meet before you MANUALLY rank it up at level up.
