I don't expect them to be in the game but it would be cool to see them in a side quest or a DLC, I am expecting the Institute to be the main villians but like it has been said I could see the Enclave being involved with the Institute but in a small role. Like giving them synthetics for technology they don't have, or the Enclave could trade technology for children to suppy their"Schools". It might be interesting to see both groups participating in a exchange at night, perhaps in a mission for Piper, I have hoped the quests she gives will be similar to the ones in Beyond Good & Evil and that they involve a camera.
Seriously though I hope there is a interesting questline involving Piper and Public Occurences. I could see some of the quests being similar to Moira's in Fallout 3 while others deal with exposing certain things about the factions. I could see the Institute attempting to replace key figures in the local communities with androids similar to Harkness and Armitage.
I really hope that we can use a camera and that the questline involving Public Occurences is interesting I think working for a post apoctalyptic newspaper would be pretty cool. We were able to use the Codac R9000 a little bit in new vegas but I hope we can use a newer model and for more than one mission.

A: The human/synth relationship cliche though it may be figured heavily into the release trailer
B: I have no doubt that the pre-war prologue features a real person(s), however if they where going to go with the "you're not really you" thing (cliche though it is) then it seems reasonable that they could have surgically altered the protagonist and implanted memories based on the personel files of a deceased vault-dweller and dumped him/her back in the vault to wake up non-the-wiser.
That is possibility I'm suggesting which does not conflict with any leaked achievements. Yes it's cliche, but we already know that the synthetic human issue is going to factor heavily into the plot line, and it's not like Fallout has ever not been heavily derivative of existing post-apocolyptic novels and film. In fact, that's kind of the point.
