Ghouls do not experience life in the same way that people do.

Their bodies regenerate using this energy. In their pre-ghoul state they have the genetic mutation that allows their bodies to convert radiation in chemical energy and they need the genetic mutation for regeneration. People without these two related genetic mutations will just die from prolonged exposure to radiation.
The necessary genes are inactive until they are mutated by exposure to radiation at which time part of the body of the pre-ghoul will have been corrupted. In some ghouls the corruption is limited to the exterior, in other ghouls vital organs are effected and either the brain will be directly damaged or the brain will be deprived of oxygen before they have become complete ghouls.
Quite frankly I'm amazed that his family didn't find him considering he's literally only a stones throw from his house. 200 years and nobody (besides family) got close enough for him to call out?
Yep just another 'filler' sidequest.
The changed metabolism of ghouls allows for hibernation with only a tiny part of the brain active for processing signals from the environment.
This is why feral ghouls often become active when somebody approaches after they have been immobile for days or longer.
So the kid probably experienced the whole thing as a slumber with short moments of awareness.
