I would say the problem is one of your mods, probably the one that lets you build Roman statues and so forth. You HAVE seen the game screen that says mods can cause issues with the game, right? Of course they've" known about it for three years"; mods can break the game.
Mods are "use at your own risk". Bethesda gives you the ability to mod your game, but they're not responsible when mods cause something not to work.
That being said, try disabling all other mods that affect settlement building. If the problem is still there, then the cause is probably your Roman statue mod. A lot of mods break the game; I've tried many that looked cool, but caused other things not to work. When that happens, you uninstall the mod and move on..
This allows you to focus enemy attacks into certain areas, giving your turrets more time to kill stuff, or to let your settlers get out and help. Having a huge open area where enemies can just shoot and blast will damage much more of your stuff that is there.
Again, knowing your settlements help. In Sanctuary i never have enemies just appear in the middle of town, i know the 3 spawn points, and my protected area is on the northern side so everything is contained outside the walls, always. Then again you have some settlements where spawn points are right in the center of a small settlement, and walls aren't going to stop such things unless you build a specific wall defense.
Walls are only useless in certain situations, or with new players who don't know the settlements and spawn points or how the spawn reacts when attacking your settlement and how to build successful walls and defenses yet.
Nope, it's as vanilla as it can get. Mods can mess up achievements, so I hold off on mods for any game until I've completed it.
I have and it has been a LONG while since anyone said anything.
There used to be issues where you ended up with duplicate cells in the settlements. You could build in them and then come back and the other set of cells loaded which made items disappear. Almost like the game forgot to save the cells.
It was also mostly a mod issue because modders were creating duplicate cells rather than editing the current cells.
Not even sure if it can be fixed.
My advice would be to not spend a lot of time replacing that stuff and see if it happens a second time. Could have been just a fluke the first time. If it does happen a second time then I would either start a new game or avoid doing anything further that settlement. If you start seeing it happen in other settlements then I would definitely run the verify files and start a new game.
With mods / no mods loaded / never ever had any mods?

These bugs aren't going away. They haven't been worked on in years, and the Creation Club is only a prop so Bethesda can legally say things are getting updated.
And if any of you thinks Microsoft buying Bethesda is going to fix things, you're in for a rude awakening. When MS bought out Mojang, Minecraft lost its soul, and critical bugs haven't been fixed in years there, either (like the settlement people changing professions and losing their maxed-out status - STILL not fixed, in years).
