I already deleted everything and reinstalled everything
if you messed around with the gpu control panel make sure scaling is set to aspect ratio or full screen .
in the falloutpref's it should look like this
bTopMostWindow=0
bMaximizeWindow=0
bBorderless=1
bFull Screen=1
iSize H=xxxx (your screen size )
iSize W=xxxx (your screen size )
and if the safe zone for the ui got played with it needs to look like this

fSafeZoneY=15.0

but the easiest fix is to delete the .ini files and let them get rebuilt , if you messed with the 2 files in the games main folder then you need to delete them and verify the game cache to re acquire those
fallout4_default.ini and in the fallout4>fallout4 folder is the fallout4pref's.ini
the game does not alter drivers , the .ini files do reset themselves on occassion , but if you just make the changes in the fallout4custom.ini file the problem is solved , and the changes you made will over ride the stock settings
