Does using the sex change command affect the game world?
100+ hours in and I've decided I would like to try and play a female character. Can I use the console command "sexchange" without any consequences? Such as will people correctly refer to me by my new gender, it won't affect the story at all, etc.?

I think the game uses "getsex" so maybe.
Try it.

Was going to avoid trying it cause I didn't want to stuff up my gameworld, was hoping somebody has already done it (surely some people have used it by now)

Why not just do a new game? Playing the game again as a female could be a whole new experience. That being said, you could always try it briefly and then reload if something goes wrong.

to my knowledge it mainly cosmetic, but it does effect companion romance interactions, opponent insults when in combat and can break parts of conversations with npcs (especially with Shaun)

Used the command quickly to see if there's any noticeable difference. It's not the same as FNV where you can change your name, etc. so I'm a pretty lady named John.

I think I'll consider starting a new game - dragging out my FO4 experience is in my best interests anyway as DLC is probably a while off still

I used it on my character. It doesn't really affect anything unless you're going back to the vault and you desparetely look for a male/female character (depends on what you start off with). I started off with a male character and I forgot that I changed the sex and I was like "when did I become a lesbian?" Then it struck me. There also has been times where when NPC's talk they get confused with your gender. I've had a few "he's" instead of "she's".

I have heard th at it can break the game/world. So, use it with care.

The only difference is people might start calling you "Caitlyn"

Oh gosh..... that's the first thing that came to mind when i saw this topic...

@OP. If it's worth anything... i remember in Fable 2 if you used a potion to change it never corrected the gender reference from the NPC's, they still thought you were what you were previously etc..

For everyone commenting so far, thanks for alerting me of the harm it can do to my save. I'm going to start a new save - I wanted to a while ago, I gave myself too many crafting mats and now I feel guilty haha. Ah well, it'll be nice to play again - I'll try not to rush it this time

Edit: Nerevarine, that was hilarious

The Answer is: YES!
When you change your player gender you lost some Perks (Due a changed specified gender) you must need to readd using some console commands.
Another thing is if you revisit Vault 111, your couple is the same sex than you.
Normally players Voice don't change...
Sometimes Father got broken and not talk with you making impossible to continue game and need to restart game from the beginning.
