and if it's something heinously egregious ( like murdering a whole bunch of Dear Hearts and Gentle People right in front of them ) they will decide that you must be removed from the universe, for the sake of All Living Things.
If that happens and you survive the onslaught by killing them, they stay dead. Forever.

This is true and thinking about it that's how Danse died the time I mentioned in my OP. These ones can die as they're now enemies rather than companions and their deaths are a part of the storyline. That said, Hancock isn't affiliated with any of the factions and died anyway suggesting it's not limited to just the factioned companions.
Yeah, the affinity scale. In the case of headless Hancock, he was on good terms with me. We even had a dialogue just before the fight.
It's worth noting that Hancock in these screens died by enemy hands rather than mine. He died in a normal, albeit it intense, firefight.
