You can equip the pipboy on your character by doing the following:
Open console
type player.equipitem 00021b3b
Only other way would be if something happened to the pipboy mod and the mod specific one you are wearing was "disabled" because the mod did not load. Probable causes for that would be if the mod has a required mod, and the required mod is having a version clash.
I suggest you check your version number and update if needed. If you already have the latest version, then you should submit a bug report on the author’s website. I believe he also has a Discord server where you might be able to find the answer.
Unless specified by the author, older mod versions should be UNINSTALLED before installing any new version. I've been adding mods to Bethesda games for more years than I care to remember and I've always found this technique to be the cleanest and safest. No doubt you'll get different opinions on this but this is what has always worked for me.
After you install the new version, load up the save you just made and check your game for stability, the pipboy issue resolved, etc. Once you're satisfied everything is as it should be, you can play on like nothing has happened. HOWEVER, I strongly recommend that you do NOT overwrite that save you made, just in case there's a problem further down the road.
That is the most annoying part of helping people out.
Part of it is our fault for not asking for a load order.
I would have suggested another pipboy mod was blocking it from loading properly, but you said you were not using any.
At any rate, the reason it works now is because the last mod loaded overwrites previous mods coding. So basically the way the flashlight mod was written was it overwrote some duplicate lines and made the mod fail.
You could go put a message on the Ikaros page on Nexus and maybe the modder can create a compatibility patch.
