That sounds more complicated that it is, so here is an example.
I rescued a settler for Sanctuary, but the kidnapped settler actually lived in The Slog. (I knew because it was the ghoul doctor I appointed/outfitted personally)
I think this is due to the fact that the aforementioned ghoul doctor was a settler of Sanctuary once, had some bonding-friendship experience there and was relocated by his master (ME!) to the Ghoul internment camp at the Slog.
After you kill the super mutant
Go to the settler and go on sneak mode
It will prompt a option saying "release prisoner"
Then you can stand up and talk to the settler
You can now recruit the rescued settler
Fallout 4 is still better than 76 and deserves to be played...
But then again, it seems that nothing will ever come close to New Vegas

No, it is a random encounter.
Kidnapping radiant quests are completely separate.
Every once in a GREAT while (like 1 in 10 of these encounters) the super mutant settler prisoner can actually be rescued and recruited. You will know the difference because you will be given a weird menu window with an option to arm the prisoner.
