Adding elements of realism like that to a series like Fallout which is already unbelievably far-fetched as it is (and I'm not just talking Fallout 4 here) seems silly.
to me it would just add a new level of hard more to a already hard early game, limiting space to carry for food & water, finding a place to sleep so you don't have negative stats or even add random event like passing out would just expand the experince.
making it optional, like new vegas, where you can pick to have it or not would just make it more fun imo
If I were to be asked, what one mod would make the game tough I'd say ammo wgt mod. You want to carry 200+ rds of like 3-4 guns, or you want to loot ammo or trash, it makes for a tougher descision then, eating and sleeping. Likely you choose encumbrance and carrying tons of ammo but that's the trade off.
