That Mechanic also apply to any Companion with personal Quests and you can't reach max Affinity with them without first finishing their Quests, you can only get the Perks for max Affinity directly for the ones that have no personal Quests.
You also reached max Affinity if they "idolize you, speaking with them then grants her Perk and you can also romance them if they are romancable and you won the flirts before.
Of course not, if you got her Quest and you get killed then, it loads the last Save and you lost everything you done without saving, that includes her Quest that started.
Ok, then simply be patient and repeat what you have done before being killed and try to not die again... If you die you loose all Progression till last save that's all, her Quest is not gone, it has never started... ;)
Yeah its annoying if you die just at such Points, but ♥♥♥♥ happens... LOL!
Next Time if you have that Conversation and her Quest starts, immediately save and you are safe... ;)
Is there a way to know exactly how much affection your companion has for you?
It's possible to push Affection for others with a glitch, if you know an NPC eg. likes Weapon Modding, you can mod a Weapon "xxx liked this" then save and reload save and mod Weapon and you get another "xxx liked this" you can repeat that till "xxx idolizes you" and then speak with them to get the Perk.
However with NPC's that have personal Quests, there are first two Conversation that must trigger and then the third Conversation where you get the Quest, once that Quest is finished you can get the fourth and last Conversation where you get their Perk and can romance them if they are romancable.
Cait was relatively easy to get there, she loves if you lockpick and i picked alot of Locks while she was my Companion... ;)
I got a similar trouble: i received enough affinity to have a conversation with Cait in which she says she is drug addict and she wants me to help her to get to Vault 95. But after the conversation nothing happens, no quest is triggered. I read about this bug at Fallot wikia, it says that the trouble can be solved by not allowing her to talk to me about her quest. Is there a way to trigger her quest AFTER the bugged conversation?

Someone figured out the solution. dismiss your companion and be next to cait then use the console command setstage 1666c5 0
this will start the quest with cait but you have to ask her to follow you again when you transport there
