Sound location problems.
Perception doesn't put a blip on your rader, so to speak, anymore, which used to be useful for stealthing. I now try to track via the sounds around me, usually within a building, but the way sounds are playing, they're often not directional, at all, the voice of someone several stories below me, can sound like it's in the room with me, or next to me, makes tracking that voice impossible. It also seems to somehow directly tie to my stealh, in odd ways, as, if I walk around normally, say, 2 stories above or below someone, they don't seem aware of it, but, if I'm sneaking, then start moving more rapidly, while crouched, they become rapidly aware that I'm around, which is... weird? They don't notice me walking around like normal, but rapid crouched movement, they do. Glitchy? Buggy? Very Annoying.

I think I'm going to ditch stealth play, till some serious patching, or more likely, some modding, comes out to fix this crap.

im having the same problem.... did you manage to come upwith a fix?

every time i hear something in the distance i try to approach it only to be going the wrong way cause the sound is really off.... it's annoying me SO much.

None that I know of, I use a surround sound 7.1 headset blah blah, which works fine in most games for sound location, but, i think they've just totally botched sound in this game. They ditched perception putting a blip on your radar, unless you're in danger etc, so, i think they made sounds stronger, but didn't make them accurate to locations.

the sound s are just another shoddily made lazy part of the game .
and they use it as a device to increase difficulty and create tension , you can hear the enemies yet you cant tell where they are or if they are any where near you for that matter, they might be down the alley and in a different building . or 4 floors away ,
this is such a poorly crafted game .
who ever was in charge never played a game , but i bet he/she has a long history in sales and marketing

I don't really seem to have this issue, the only times I have any issue with using sound to find enemies would have to be when they are on a floor below or above me and are right near were I would be on that floor, or when get surrounded by feral ghouls and the like (but I think that may be more due to the mass of sounds happening at once).

No, I get messed up with the weird way sounds work as well. I just ignore it and figure the computer will react to anything within 10m (in all directions) and trigger a vocal response that doesn't recognize barriers like walls, floors, etc.

Usually, when that happens, I just stand up and let them hear me better. I can hear where they are much more easily with footsteps and doors opening, etc.

Vertical sound definately travels far worse than horizontals. I can follow the sound of gunfire outside to a fair degree, but yeah, indoors, in a multi-storied building, the sounds up to 2 floors above, can sound like they're in the next room, it's very disorientating.

As for the dramatic music, I dialed that way down ages ago, it would stupidly play during dialogue and as I didn't have subtitles on, a lot of what NPCs said to me kept getting drowned out.

I know this was a long time ago but my sound issues came from one save only. Sorry for the late reply.

Once i started a new game the sound was fine... quite unusual.

Yep, it seems indoors at least environment has zero effect on audio, walls, floors, doors and objects dont change how the sound works, might as well just be in a large empty warehouse as far as the audio engine is concerned. Sad because this used to be a thing in PC games and somehow we have gone backwards.
