Fallout 4 is probably the best sound design out of any past Beth release.
I don't know what it could be for ya, and I don't mean anything by this, but could it be something you are overlooking? I'd have to say it's probably on your guy's end.
Sometimes when I start the game and switch the headphones to Stereo, ALL the sound is in the right speaker, other times when I start the game, and I switch the headphones to stereo the sound will be correct.
I don't bother with Realtek Audo manager when using the headphones, because they're USB
I have the RealTek audo manager set to 5.1 for my Surround system because it's 5.1, but I never use it because it's too loud for my neighbors.
