They're literally functionally indentical to a maxed players Stealth skills in a handy portable device - I believe in Fallout 3 and New Vegas it was instantly +100 to Sneak or something. It didn't eliminate footstep noise but it made you extremely hard to detect...practically just an aid for difficult situations in quests if your Sneak skill was terrible.
They used to great effect by enemies in combat but when you use one the enemies know where you are and just reign in on you. It's how stealth has always worked though.
