If you're having trouble getting within melee distance while sneaking early on, you can use VATS to quickly close the gap and get the sneak attack bonus, just make sure you won't be seen by someone else if you're in a crowded area. It doesn't make you leave stealth once the animation finishes to make sure you don't hit the button again afterwards.
Not sure how true all of these are, but at least some of them will help: Reduce how much you are carrying, get out of your power armor, strip down to basic clothes (or stealth upgraded armor), get some magazines to increase your sneak, and get yourself a good high damage suppressed weapon. Stealth boys can also help.

Your armor weight affects the noise you produce. Muffled armor pieces elimate the penalty for that piece.
Even maxed out AGI, perks, and a chameleon chest armor means I still get detected if I'm in a well-lit area, in front of some bad guy. Which is really as it should be. But sneaking up behind someone's pretty easy now.
