Wish stealth wasn't so useless.
Enemies can still detect you easily from miles away. This makes melee weapons practically useless.

I'm completely undetectable and I wear heavy combat armor. You must have really low sneak skill I'm assuming because at rank4&5 with the addition of stealth ops magazines you are a ghost. Do you play with your Light on or something, or Stealth run without rank 4?

You still need to account for light & shadow and what type of armor you are wearing. Your companion will never be as sneaky as you - they can bump into doors / objects making noises. Super mutants & hounds can smell you.

Also playing stealth on survival. I reached a perk level where walking isn't affecting your camouflage. At this point you are pretty much invisible (i think it is rank 4 stealth). stealth is really bad at lower levels, but once you reached about 3 or 4 it gets really good.

get the mod that remove the level limit of perk.You can be perfect stealth very soon

I have the 4 books but only 4 agility and nothing in stealth perks but I still get in close enough range to snipe or shotgun enemies down.

Finally got two decent silenced weapons and the game is finally getting easy on survival for me, level 25 now.

my problem is with companions being terrible at stealth. i can't count how many times i have stood right next to my spotted companion and took "sneak" shots at the enemies that are fighting them.

I have walked almost into some with the light on and not been detected. Often in the ghoul places if I'm paying attention to their locations I can walk almost on top of them even out of stealth- much easier to get headshots when they're not alerted and rushing toward you in hordes.

I found myself crouching next to a deathclaw and another just over the hill that I was actually sneaking up on, never saw the one I was next to until I saw its tail move, freaked me out.
Companion bumped into the deathclaw and it was on.

Also at some point you can mod your power armor to have stealth boy on crouch, there's a legendary armor mod that has some sort of invisibility or something when you're not moving while crouched too.

10 Stealth tips:

1. No companions. None of them have sufficient stealth skills to be useful, they will only get you caught.

2. Lone Wanderer perk. See tip 1.

3. Wear light clothing. It is possible to perk up to the point where you can wear heavy armor and still be practically invisible in low light, but that comes later when you are in the mid range levels and can perk up stealth.

4. Join the Railroad to get the ability to mode regular clothing with Ballistic Fiber, you will be glad you did.

5. Agility does matter, but not for what you think. Having 10 agility doesn't guarantee you to have an easy time sneaking, but it does unlock the perks you want. Find the useful ones and scale your agility accordingly.

6. The Covert Operations magazines are a big help for staying undetected as they give you a free boost, but they are not exactly easy to find.

7. Believe it or not the armorer perk is useful for sneaking because you can mod your armor to be "shadowed", making you harder to detect no matter what armor you are using.

8. Light and sound matter. Unlike some other fallout games you can't be standing under a floodlight and be considered invisible if your stats and perks are high enough, you actually have to sneak.

9. Perk up sneak until the act of running doesn't affect sneak detection. Things get 200% easier.

10. Obvious, but silenced weapons. Either find one or get your gun nut skill high enough to build one.

He meant 'level requirement'. There's a mod that simply takes those away, meaning that you don't need to be, say, level 50 to max out Pain Train, or 41 for Science 4.

Yeah, kind of a pain to have to level up the SPECIAL skills to the necessary rank AND level up to a high level to unlock certain perks :\ It does make the game more difficult, though. Too many mods like that and the game wouldn't be much fun anymore.
