95% Headshot in VATS

Hi all,
for some reason I have a 95% headshot chance at any range with any gun when using VATS. My perks didnt show something like that. I did not use the console nor any cheats at all. I couldnt figure out why that is, neither of my perks says something like that nor does my equipment say something about this.

I would like to know what causes this and how to avoid it (right now I try to use VATS for other bodyparts instead of head, because its far to strong/easy with it).

I heard something of a companion perk. Do you have the mercenary from the bar in goodneighbours with in a good relation to you?

maccreadys vats increase perk is pretty significant do you have that unlocked?

Yeah, I have that perk 8even after I send him away). The perk just says additioal 20% not 95% all the time.

The perk is bugged. It currently provides a 2000% increase, instead of 20%. So shots reach the 95% accuracy cap no matter what.
