I think it happened while I was turning in his last quest, I remember spamming my keyboard a lot to skip the banter, maybe that caused it? I also tried disabling and enabling Rhys via command but without success.
Anyone got a fix?
EDIT; It fixed itself after doing a few runs for Scribe Haylen only
This glitch affected me also, i'm almost postive it was from spamming through Rhys' talk as well. I can confirm that completing a few Scribe Haylen quests will make Cleansing the Commonwealth available again. It took me 6 missions.
I can also confirm Scribe Haylen is far more pleasant to work for than that douche Rhys :P

Once you move past a quest point in a faction, you lose the ability to quest for the other factions.
Did you complete the intro quest with Danse at the police station fending off the ghouls?
I sided with the Minutemen for the main quest and ending and I am still doing the 4 radiant BOS quests over and over to max out companions. I have done 107 for the BOS and 179 for the MM and all the RR/Institute quests until they got mad/blown up.
