If indeed extensions are not shown, you have to change that in your settinings.
Otherwise - I don't know
I have that file, but its not the ini, nearly no text in it at all except for some minor visual things

not pirated, private profile keeps the russian scammer spam out, i figured this issue out a while ago anyway, it had to do with having fallout installed on a different HD than steam, the folders just got put in a wierd location for some reason.
I'm using Windows 7
In the game install directory is Fallout4_Default.ini, don't bother changing that file. Look under Documents\My Games\Fallout4\Fallout4.ini and make your changes to that file.
I had to add bInvalidateOlderFiles=1 under [Archive] to get scripts I'd written in Creation Kit to run....
Fallout4.ini
Fallout4Custom.ini
Fallout4Prefs.ini
It seems if one of these goes missing that NMM looks at or wants to edit it will complain about the .ini file being gone.
This actually helped me find my .ini file lol
Oddly enough it just doesn't show .ini on it but still has the info i needed, so thanks regardless!
