There's this thing called the 'Continental Divide' that makes travel between the west and eastern halves of North America difficult. Plus, at this time, the NCr is *blocked* from expanding eastward by the Legion and the Legion is tied up trying to hold on to its territories in the west against NCR expansion.
The Legion originated from the consolidation of tribes east of the Colorado River. The Legion isn't located anywhere in California. They never get farther West than Las Vegas.

There is a new faction. It's called the Commonwealth. It was mentioned a few times in FO3 if you remember, but now we get to see the entire faction rather than simply hearing about it thanks to the game being set in the Commonwealth's home territory, near what was once MIT.
Console's gonna have mods too this time around. Now the main advantage with PC is the number of mods. It's unlikely that console will get to have script extender mods, that's something that PC will most likely always have over console. That and sheer flexibility and power.
