+ Try disabling steam overlay VR and cloud sync under game properties. in steam, click:
LIBRARY -> HOME -> FALLOUT 4 -> MANAGE -> PROPERTIES -> GENERAL ->
uncheck the boxes that say:
- use desktop game theatre while steam VR is active
- keep games saves in the steam cloud for fallout 4
another thing you could try is install F4SE. it has a keymapper file that you can use to redefine keys. i wrote about it in my guide on the mapping keys section (actually removed it to place into a new guide so i can't link it right now)
It is possible to remap keys in Fallout 4, including the console key. I once was running forward and backward using mouse clicks in build-mode without having it spin random objects as I passed by them.
The easiest solution to rebinding keys is F4SE

If you're really in to modding then you should already have it installed anyways. When you install F4SE it comes with a file named "CustomControlMap.txt". You should copy this file into the following folder (or to wherever you installed the game to):
C:\Program Files\Steam\steamapps\common\Fallout 4\Data\F4SE
You open CustomControlMap.txt in your favorite text editor and find the line that looks like this, for example (if we're customizing the console key):
You then need to replace the "0xc0" bit (or what ever is there) with the VK_OEM code for the key you wish to use. This link should help you find the one you want.

If you want to go back to the original key bindings then simply copy/paste the "CustomControlMap.txt" from the install folder to the "Fallout 4\Data\F4SE" folder again and overwrite the one you have been editing.

It sounds a bit like you have an Action Set Layer overlaying the B or all the controller, and it's mapped with the overlay key to be exit. Most configs do that with the start mapping. You might also have a Steam chord set up...
Did you config your controller yourself, every button, or did you let it auto-download a config, or pick one from uploads, community, etc?
You can always gakk it and start over; steam, settings, controller... the overlays are in either bit picture or desktop, and chords are in, well, chords.
I was able to turn off desktop overlay, and deleted the O/B buttons back binding. Still no luck.
I just used a community config new to Pc gaming thought that would be the best place to start
Take heart, it's something easy we just got to figure out which one it is.
