I am working on play thru #4 now and all 4 times the Church door had a marker, while hunting the SM's they all had markers, and when they were dead, the quest told me to return to the quest giver, and it completed quite nicely all 4 times. And I do not have any patches on my game (GoG player).
Your "obvious" conclusion about the "claims" that I and others have may not be correct.

It was a radiant quest at University Point, no quest marker. That made it difficult because there are so many buildings and floors and the outside area with shacks.
I killed every ghoul I could find but the quest did not complete. Then I had a Help Defend Checkpoint mission. I did that and walked back through the water. I tripped on a Glowing One that was under water. He stood up and the quest marker suddenly appeared.
I found a solution for my problem, "Clear the Super Mutants from Trinity Tower". The quest was from Diamond City's Most Wanted side quest. The problem was that I had already been to trinity tower area and had cleared all the super mutants, THEN I went to diamond city and grabbed the quest after that and the quest was stuck in miscellaneous and couldn't be completed, and wouldn't show any marker on the map.
Solution:
I opened the console and typed: setstage 18127d 100
I was given caps and the quest completed.

The problem there may be that the quest can select its details when the location was still occupied, but you clear it out before triggering your own access to the quest. If so, it means someone forgot to include a check for that possibility in the quest design.
It's less of a problem in other cases because some of the radiant quests time out and fail with no lasting consequences.
