What the OP showed is Normal in the game. It ain't easy seeing underwater. Focusing your camera at a slightly downward angle when at the surface of the water does clear the water to see better. That is also normal.
There are nexus mods to clear up the underwater view.
*There might even be an .ini setting to clear it up but, I don't know which one it might be.
**Incase you don't know, there is nothing underwater in this game that will attack you.

If it wasn't for this optimization would the game have to render part of the screen with normal shaders and the other part with underwater shaders, which makes the engine a bit more complicated and slower. It's certainly not perfect and can even be annoying in an unusual situation, but one does usually transition through the threshold rather quickly, which is why Bethesda didn't deem it necessary to make this perfect.
If you have modified FOV values then this can contribute to the effect.
On a note, some of the visual glitches actually don't originate from the game engine, but from optimizations within the graphics driver itself, which makes similar assumptions and then takes similar short cuts in order to gain performance. So you may for example see objects or shadows disappearing at the edges of your screen and not only with Bethesda games but in other games, too.
I have also seen another person with this same exact problem, a Youtuber named Oxhorn. It's just a terrible muck. I may have considered it natural for the game, but then, there are areas that are clear as day as depicted by the first screenshot I have taken, and there are areas that are not. And that is just by slightly tilting the camera angle up and down.
