Depending on perks and your weapon, it's fairly accurate at medium-ish ranges. Far out, you better be using a scope. At closer ranges, you're better off switching to first person. If you're aiming for specific body parts, your best bet is first person sights/scope or VATS, depending.
I mostly play third person outdoors and first person indoors, run around with Power Armor and a range of different weapons, generally Big Guns and Autos. Most times in third person, I just aim for center mass or a little higher, and it still gets the job done. But that's also with maxed weapon perks and fully modded Legendaries sooo...mileage may vary. >_>

Hell, I remember people calling 3rd person cheating in MW2....like lol...wtf...its part of the game lol.
Easy sure...cheating...far from it.
I can't hit a raider in the head 25 meters away with a pistol or an assault rifle now. It sometimes is dead on, sometimes completely misses.
I'm lvl 73, with rifleman and gunslinger maxed. I don't use VATS, just not my thing.
I'll probably just go back to first person. I don't like mixing up the views, I either play a game all third or all first, don't switch back and forth.
I went into the Boston Preparatory School and killed all the raiders there with pistol headshots, no issues. used the assault rifle on a few and it kept missing at the same ranges. I just feel like the aimed in mode with third person isn't nearly as accurate with ironsights.
So all those benefits of 3rd person aiming VS 1st person aiming in Fallout 4 is a handicap and akin to cheating . So we all must be 3rd person to even the playing field again. Fortunately Besthda knows 3rd person is inherently easier and they made it more inaccurate, nice job.
Go near to a wall, maybe 20-30 feet away, and fire while aimed at the wall in 3rd person.
If the bullet is hitting the wall a large distance to either the left or right of the crosshair, then alt tabbing might fix it.
Other than that, not to sure what could be the cause.
I personally use a 45 pipe revolver rifle with iron sights, and it is very accurate in 3rd person. Misses occasionally at extreme ranges (the kind where you can barely see the target) when I aim for the head.
its no different then cheating with sneak mode, when it goes from hidden to caution then ya know enemys are around and not to mention doubles the damage of weapons :p

Handicaps are used to even the playing field...by your logic here....handicaps are cheating.....
No because every player can sneak with equal advantage and opportunity. Say you were playing a PVP game and it was more challenging in 1st person than 3rd person. Would you feel your opponents score who solely uses 3rd person is as justified as your score if u solely used 1st person? Hell no :)
