audio sync issue --- again!
So I've had this problem before and I fixed it for the last several days. Today, it started again - and I changed no settings at all.

I have iPresentInterval=2 in my fallout4prefs, and fallout.ini files. I have my monitor refresh rate set at 120 hz.

But, once again, the NPCs audio lags about 1 or 2 seconds after their lips start moving. Same if firing a gun: see it, hear it over 1 or 2 seconds later. I have no idea why this cropped up today for no reason. Any ideas?

Check for any power saving features on the audio port, meaning, Bluetooth or USB or however it is connected, and disable it.

Open the Device Manager (Windows Settings->System->About->Device Manager) and go through the list of audio devices, see if you can find it there. Then open its properties and maybe there is an option where you can turn off power management for the device, i.e. do not let Windows put it to sleep.

FIXED! Somehow my Fallout 4 launcher got flagged for Windows 7 compatability mode. This was an issue before - not sure why it reverted.

I go through hell every time Windows updates because it messes with a bunch of settings and changes them to something other than what they were, especially for sound cards.

It is out of sync because your FPS is too high. You need to lower your FPS or get a mod that creates a sync for them.

There are a couple of reasons for this.

1.) The frame rate is too high and causes dialogues to cut short and NPC to talk over one another.
2.) Power management suspends the audio stream and on wake-up introduces a delay.

The first can be fixed by limiting the frame rate or using the High FPS Physics Fix.

The second can be fixed by choosing a different audio device, turning off power management, updating audio drivers, disabling audio enhancements, etc..

Not sure why Windows 7 compatibility would factor into this as the game is not aware of Windows 10/11 features. If anything does Windows 7 compatibility allow the game to integrate better with Windows 10/11.

You guys should add a Note(s) to the Library View for fallout 4(games).
Before anyone forgets "again". That way you will remember if you ever uninstall any game really... very handy if there are steps other than just clicking PLAY games that need nothing.

Unfortunately, after the update, I'm having the sync issue again. Bethesda told me to do this, which I did, and it fixed things for a while but now there is about a second of lag between action and audio cues. https://help.bethesda.net/?linkId=100000258581205#en/answer/41680

It helps with the refresh rate. My monitor is at 120hz, and ipresentinterval=2 on both the .INIs - not sure why it doesnt work again.

Turn on an overlay with an fps counter. Start there and check whether you actually are getting 60fps.

Then try a different audio device for a test and also disable audio enhancements in the Windows sound settings for the audio device. See if this changes anything.

i not sure of your issue cause. i had game crashing sometimes after I installed Boom3d audio equalizer... & had switched in windows from speaker output to High def Nvidia audio driver... i'm guessing too much to ask of the vid card or system ... after putting back to audio speaker output it's fine. Maybe this will help you & or someone else. ;-)

Okay I figured it out - I usually play my games with a wireless headset. For some reason, there is a super high latency with this game. I just plugged in an old wired headset and everything was in sync.

My wireless latency is good for Youtube viewing and all other games - for some reason it's having a sync issue with just Fallout 4. Any ideas? Thanks.
