What did you set your graphics resolution on the game launcher?
What frame rate?
Using Nvidia app or Geforce Experience will simplify the in game settings.
You can also open the properties to Fallout4.exe and Fallout4Launcher.exe in the Steam folder, right-click them and open their Properties, then go to Compatibility and at the bottom can you set a High DPI Override. Set it for both to "Application". You can then keep using a scaled desktop and the game will receive the true display resolution.
