When launching this game it doesn't automatically switch focus to the game. But it does lock the mouse in place. You have to alt tab into the game before it will work.

Once playing this game, if you alt tab the keyboard stops responding with the player. Inside of the menu it works fine. But once you control your character again , you can't get back to the menu without alt tabbing out and back in.

Do you have a controller or other input devices next to mouse and keyboard connected? If so try unplugging them and see if it works better.

If you are playing on a laptop then make sure not to double-click the PLAY-button in Steam but to click on it only once. If you have a mouse pad then you could also try connecting an actual mouse to your laptop or you can try starting the game directly from the Steam folder.

There is also an alternative launcher, Bilago's Configuration Tool. Next to being a tweaking tool for the game's .ini-files can it also launch the game (or SKSE) and even allows for a custom background so you can have a personalised Fallout 4 launcher.

Alt tab and this game on steam is hit and miss. For some people alt-tabbing actually fixes slow loads and some other issues. In many others, it tends to get really janky and leads to resolution issues and eventual freezing of the game.

The better solution is to not alt-tab.

Typically normal, intermittent issue.
Have you tried windowed mode? That works for me.
