It comes out that you have to turn "Wait for Vertical Sync" from "Off, unless app..." to "Always OFF".
Now I can in 86-120 iChill range again.
now i cant go over 37fps again...
Special K help and it works, but I still have between 1k and 2k fps in menus
Manage 3d settings > Program Settings > Fallout 4
refresh rate = Highest available
Vertical sync = On(if you have gsync or freesync
Frame cap = To your liking, I used 120 before and game looks smooth as butter but I then lowered it just to reduce the GPU load.

Yes it works for me aswell, (the ini file thing doesnt work anymore) but menus are still on 1.5k fps +++
