NPC voices gone?
I've got dialogue scenes with my player & NPCs just looking at each other and bobbing their heads with no voices coming out. I do have dialogue volume all the way up. Anyone else experiencing this?

Check audio settings in game.
Check if you didn't change audio output.
Check Audio format for your audio output, under Sound settings. It shouldn't exceed 48Khz, set highest bit possible(it doesn't matter here)- 16/24/32 48Khz.

I can hear other sounds in game like the music and sound effects, just not the voices. I don't understand how changing windows sound settings will fix this, I've never heard of that.

Try restarting the pc if the game was just installed? Fixes stuff sometimes?

Anything in the documents/my games/fallout4/fallout4prefs settings file that might be useful?

Can you save the game? Files locked from changes?

Maybe something is wrong with the language setting.
Check Fallout4.ini
sLanguage should be en

sLanguage=en

Make sure you run the game at Minimum 60 fps depending on your card.

My sound is set to 192 KHZ, the game is played at 48 KHZ max anyway. But it helps in some games to put it at a MINIMUM of 48 KHZ not 44,1 KHZ.

There is some major audio bug with this update and its something to do with screen resolution and upscaling. I get audio at native resolutions 3840x2160 @60 hz. In game it cuts in and out. No npc voices or nothing at all. Sometimes I can restart and get audio buts choppy as hell. I usually game at 2160, so changing the resolution to 2160 and using FSR to upscale on andrenalin, the game launches in a 2160 window with audio but once I mouse click and the screen upscales the audio drops out completely. Bethesda are criminal.

For me all sound from other NPCs and background sounds decreased in volume because my system was set to 192KHZ.

I've tried a myriad of different solutions & nothing has worked. I'm going to chalk this down to just a good old fashioned bug. Is there any way to submit bugs to Bethesda directly?

this is a mod or language bug, the only fix i found possible is to delete EVERY fallout4.ini file you have, so it replaces it with the vainilla one, and the npc voices should return (in my case i had to delete the one in "my games"folder on my documents and then the one in my modded mod organizer "profile" folder) example my games location : C:\Users\username\Documents\My Games\Fallout4, and MO2 location depends on where you tell it to install, in my case its like this D:\Mod Organizer\Fallout 4 Full Modeado\profiles\Default hope it helps

Are you playing with a lot of mods?

I ask because I have this issue at times, but I also know what it is in my case.

I may case, I run a script heavy mod list. What happens is the papyrus engine in Fallout 4 get overwhelmed trying to update the scripts that are firing off and the play voice script gets lagged up.

Everything else works fine but 2 things: No voice files play, can I can't speed up the dialogue. Background noise is playing, guns shots go off, music on the radio plays... just no the voice files.

In my case I just have wait a couple minutes for the scripts to finish running and the voice files play again.... until the next time there is a script dump.

Have you tried entering "fks" into the console? This resets the game's sound system.

Did you then make changes to the game or the PC recently? See here under Audio Issues for Windows related problems:
