Report a translation problem
Had to change scale to 100% in windows screen setting for it to even display resolution properly but then it starts in small window.
Tried changing ini file - no result.
Bethesda always knows how to surprise us.
Try to delete your "Fallout 4" folder under my documents/my games if you had it installed before. That fixed it for me because messed with some ini files when I last played the game a year ago. Probably set some files to read only. Hope that helps you!

I forgot to mention that I also forced both the "Fallout4.exe" and the "Fallout4Launcher.exe" to start with admin rights. And I deactiveted the fullscreen optimization and changed dpi mode to program/app. This plus deleting the savefolder and that did it for me.

The main reason is in -----OneDrive SETTING -----. You should complete setup of Windows OS by installing OneDrive, sign in there, confirm folder for OneDrive, complete installation. After that you can unlink OneDrive to stop syncing. But after you set settings for OneDrive, Steam starts to create Fallout 4 folder in correct Documents folder and not in One Drive/documents folder. And everything works fine.
The reason is that after installing OS a lot of users skip that OneDrive setup and settings and don't login into OneDrive on Windows. And this messes up those documents folders where setting file for games are saved. So don't play with registry - just complete setup of OneDrive in your Windows and then stop syncing files if you don't need it by unlinking. Fallout 4 will work in Fullscreen as you selected in Options in Fallout 4 launcher. Hope this helps.
