fixed it for me.
Set windows scaling to 100%
Delete the " Fallout4Prefs.ini " file
restart the game and then it should run the gfx-setup again.
My fps went up to 100+
Dont know if you need to set scaling, but had to to get it to run UW resolutions properly (was not able to select it before).
Example:
C:\Users\YOURUSERNAMEHERE\Documents\My Games\Fallout4
Fallout4Prefs.ini
If it doesnt, put it back.
What currently works is this:
1.) Open the Nvidia Control Panel
2.) Select the 3D settings
3.) Open the tab for the individual applications
4.) Find Fallout4.exe in the drop-down menu
5.) Enable low latency mode for the game
5.) Set a frame rate limit for the game with a max of 60fps
6.) Force vsync to "fast" for the game
The game is currently behaving oddly when it comes to frame rates and high refresh rate displays. iPresentInterval in the .ini files is broken at the moment. So one has to force it through the driver.
This will give you 60fps, thereby keeping the game's physics in check, all while you get tear-free rendering with a high refresh rate display.
This works only in fullscreen-mode. In windowed-mode it still acts weirdly.
An alternative is to use mods where you can keep the driver's default settings for the game.

If you are not sure about mods, but want to try them, then first find a good modding guide and read it or watch it.
