iPresentInterval=0 doesn't work anymore
I installed the game today before the update dropped and it worked just fine. Now the update drops and I'm locked to 48 fps and every time I change it in the .ini file, it changes the value back to 1 when the game boots. Nvidia vsync options don't seem to do anything.

Just an idea, have you tried making it read-only?

Change it back and make the .ini files read-only.

I did, didn't affect the game at all.

Then you will have it enabled in the graphics driver. You can force it off in the settings to the graphics driver, too.

Try Changing the iPresentInterval=0 value in both the Fallout4.ini & Fallout4Prefs.ini

Read my post and you'll see that I already tried that.

Then you are doing something wrong. A game cannot force vsync on when the driver overrides it and sets it off. Driver settings supersede game settings.

You have to play this game in Fullscreen and disable G-sync/Freesync/V-sync to have more than 60 FPS

Yeah, it works in exclusive fullscreen mode as it always has. But windowed and borderless don't work even though it worked before the update.

Works fine using Fallout4 Config tool by Bilago, iPresentInterval is still set to 0

Is anybody know how to open launcher now?

Jesus, this company still manages to amaze me. I have the same issue. Game worked just fine before the patch with iPresentInterval changed to 0. Right now it changes itself to 1 every time i launch the game. Doesn't matter if i click on a launcher or an .exe directly. Read-only is not working - i mean the .ini file stays intact, with iPresentInterval=0, but the problem is still there. No idea how to fix this

This!
And set both files to read only.
