What missions do you have listed on your pipboy? Including Misc. missions?
You don't have to immediately go back anywhere. Speak to plenty of people. Make sure the quests are active in your pipboy.
The network scanner is the very first thing to do in the institute, so... no, that's definitely not it.
It might help fix this person's issue but it's not a normal thing, since you are in fact *supposed* to put the network scanner in the first available computer device, which is literally directly outside the room where you are teleported inside.
Yes zekiran, I believe I spoke with shaun and he said all he was going to and was meant to update my mission, I then didn't realise something was wrong so had a long wander, while overwriting my autosaves. I believe I then did the network scanner. So I dont think scanner caused my issue, i think a general glitch did. My current mission I am trying to complete is institutionalised. It is now showing as having all objectives complete, but mission is still active. It has not given me the task of finding the heads of department.
If you're NOT on a console and NOT using survival, you could use the console to figure out what stage your mission is at, and maybe nudge it that way.
If you CAN use the console: SAVE your game manually first. Even "broken" it's best to have a hard save point just in case.
Open your console (the `/~ key on most qwerty keyboards)
type in
sqt
That is "show quest targets" and will bring up a list of all of the currently available missions you have.

Your quest "Institutionalized" should be MQ207.
I'd suggest then clicking on that quest's linked name, on that page, and read the chart of information for its Stages.
In your console then, type
getstage MQ207
and compare it to that chart. If you really can't find the leaders, or if you've got some other number, you can skip bits CAREFULLY, by typing
setstage MQ207 [add your number here without brackets]
Likely either 160 or 150 where you may be 'stuck'. You don't want to do this often or lightly because every time you mess with the quests there's a chance more problems may arise.
If you've got any mods even trivial ones they may also be interfering with this (companion mods, etc).
