Vault 81 Cure
Saved the game, and then kept the cure for the first time ever.
It really doesn't change much. You don't get the useless broom closet in Vault 81. Hangman alley is a better base than the closet in Vault 81. Don't get the syringe shooter. Big deal, never used it. Austin's dead. Yeah, never see him again either alive or dead because not much reason to look for him if he's alive. Don't have the annoying Mole Rat disease. Vault 81 are a bit more rude than usual, which is pretty rude.
Not so bad. I was going to reload the save and do the right thing, but I became curious what Diamond City Radio says about it.

In real life,
1) both Austin and SS will spread the disease regardless of "the Cure", depending how the disease is spread. We went over this with the covid shots.
2) half a dose for a child is about right.

If every moral choice is clear cut, you don't understand the problem.

Assuming that catching the disease automatically targeting the Sole Survivor when a molerat attacks anything (including other molerats) is a bug, I'd say it's most likely transmitted by exposure to infected bodily fluids, and passes from molerat saliva to the target's blood when they bite. Alternatively, it could be entirely blood-borne and is spread to the target thanks to cuts and abrasions in the attacking molerat's mouth, which would rule out COVID-style infection by breath-borne fluid droplets.

Either way, living in the relative safety of Vault 81, Austin could probably be quarantined effectively (indefinitely or until he's no longer infectious), but I don't think the same could be said for the Sole Survivor as they go back out into the wasteland without a cure to face constant injuries, and sleep with multiple partners.

Since the Sole Survivor has only just been infected and is yet to develop serious symptoms, there's a reasonable chance that giving them the cure will exclude them from becoming an infection risk and spare the wasteland from a plague outbreak.

Not that I imagine it matters so much for Vault 81; they can just seal the doors if the surface is ravaged by an incurable disease.

Be sole survivor.
Get bitten by a mole rat. Get mole rat disease.
Go fight ferals. get bitten by feral.
Ferals bite others.
fight normal mole rats.
Normal mole rats bite others.
Get bitten by blood bugs.
Get bitten by rad-bears.
