Then go into Steam, right click Fallout 4, click Properties > Local Files > Verify.
Wait until done. Then relaunch the game.
If you have any face or body mod(s) that were turned off, check those pages too.
The problem you have is the meshes added to the game by body mods aren't removed by normal uninstall, and game will still use those replacers. You could uninstall / reinstall the whole game but that's a long and inane process so save that for last option. Deactivating doesn't remove the replacer mesh.
Coomers ruin everything.
Many of them have an option for nude or never nude so if you have issues with seeing nude npc characters in a game pay attention to the description, pictures, tags, and options a mod has before installing it most will be clear in their description about its contents.
The most that would need removed would be the sub folders in the data folder that the replacement was installed into and/or the ba2/esp/esl files the mod installed.
Then verify the game files and reinstall mods that you want to keep.
No need to remove the entire game folder.
At most looking at the view contents option on its file page should list all the file names that need removed from the game/data folder.

Most mods that replace the body in game will put the files either in a ba2 file that will start with the name of that mod or as loose files found in the folder data\textures\actors\character\basehumanfemale or data\meshes\actors\character\characterassets
if you know what mod installed them just go to its nexus page to the files tab then click preview contents it should show all the files the mod may have installed.
Delete ALL files and folders inside.
This will take care of any loose files.
Then verify game to gain back only original game files.
